Vision benefits are managed by Spectera and other companies. We will disclose information about service provision in accordance with the terms, provisions, limitations, and restrictions of the policy, as well as other changes in the condition.
In-Network Provider Services
VisionWise programs use the UnitedHealthcare Vision Network. You will benefit from your insurance coverage by engaging with our network of ophthalmologists, including not only local doctors but also visiting specialists. You will not need an ID or other documents to receive services from an in-network provider. This makes the process of receiving medical services even more comfortable and fast.
Out-of-Network Provider Services
If you choose to see an out-of-network provider, you will need to pay the full cost of services upfront. However, you can later submit a claim for reimbursement of covered expenses. This gives you the freedom to choose and the opportunity to visit specialists you trust, even if they are not included in our network.
Exclusions and Limitations
As with any insurance program, VisionWise has its exclusions and limitations. Important note: this is only a general list of exclusions, and for complete information on insurance, you should review the policy. Some exclusions may apply depending on the state. Purchasing this program is not contingent on purchasing any individual or group medical insurance.
Potential Vision Costs Are Not Covered in the Following Cases:
- Any expenses or services not specified in the contract.
- Ophthalmological examination services that are not covered by the ophthalmic equipment provider.
- Services for which the insured person may receive payment under compensation claims or other laws requiring the employer to pay.
- Ophthalmological services provided free of charge to the insured person without insurance coverage for the costs.
- Services exceeding section or benefit limits as specified in the contract.
- Services provided by an ophthalmologist who is a relative of the insured person.
- Services provided before the effective date or after the agreement to temporarily continue coverage.
- Over-the-counter products.
- Lenses exceeding standard sizes.
- Replacement of glasses or lenses provided under the policy that are lost or broken.
- Medication or surgical treatment of the eyes.
- Fees for missed appointments.
- Eye exams or corrective eyewear required by the employer as a condition of employment.
- Corrective treatments of an experimental or investigational nature.
- Sales taxes on vision care services.
- Corrective surgical procedures, except adjunctive ones.
- Orthoptic or vision therapy training and other forms of training.
- Additional glasses except prescription glasses.
- Additional lenses.
Program Conditions
It is important to understand that the list of exclusions is only a general list. This is not an insurance contract and not part of an insurance policy. Purchasing the VisionWise program is not contingent on purchasing or applying any individual or group medical insurance.
Eligibility for the Contract
The VisionWise program can be issued to the primary insured person aged 18 to 99 years or to a spouse/partner (as defined by state law) aged 16 to 99 years. This is a wide age range, making the program accessible to most families and individual consumers.
What Happens If I Enter an Incorrect Age When Applying for Insurance?
If the age of any insured person is entered incorrectly, our records will be updated to reflect the correct age. Insurance rates will be adjusted to reflect the correct age, and any overpaid amount will be refunded minus any benefits paid by us. Coverage will be canceled from the effective date of the agreement.
What Happens If IChange My Place of Residence?
If you change your place of residence, we ask that you notify us so we can update your information and adjust your insurance coverage according to your new place of residence. This is important for providing medical services and calculating insurance payouts.
You may pay more if you use specialists not included in our network.
Out-of-network specialists may charge you any amount. In-network personnel agree to discounted prices and covered expenses without additional charges, except for copayments (if applicable).
Premium Changes
We reserve the right to change the rate tables depending on the class, as provided in the contract. No later than 31 days before the new rate schedules are implemented. Each premium will be substantiated in the rate table applicable to premium payments.
Renewal and Continuation of Insurance
The contract is renewed until the earliest of the following events occurs:
- Failure to pay premiums on time as specified in the contract.
- End of the premium period following your request for renewal of the contract.
- If you commit an act constituting fraud.
- If your insurance or all insurance in the state is terminated.
